The original form is a white (8.5 by 11) piece of paper you received after successfully completing the course. This form waives the driving test at the DMV. If you lose this form and need a replacement do the following:

1. Send a self addressed stamped envelope and $20 to:

2. Include your complete name, date of birth, and date you took the class.
